Shipping Label

This printable label is provided for free shipping of your cases to Sun Dental Labs. After you have completed your prescription form (and enclosed it with your case in a sealed shipping box), download and print the (proper country's) shipping label, fill in the pertinent data/address information and trim the label along dotted border. Then, using transparent tape, affix the label to the exterior of your case package. Sending your cases to Sun Dental Labs this way saves you both time and shipping expenses. (PDF 98KB)
